If you would like to complete the enrollment form before you come in, click on the pdf file link. This gives you the option to print or e-mail it. There are other forms that will need to be completed, and if you are under 18, you must have your parent or guardian come in when you come. This is a requirement of enrollment if you are under 18. Ideal registration times are from 8:00-9:00am or from 2:30-3:15pm, but we can also schedule to meet with you on Monday and Thursday between 4:30-7:00pm at the public library.
